Terms and Conditions of Service
1. Introduction
Welcome to Care Holistic. By accessing our website or using our NDIS support services, you agree to comply with the terms and conditions outlined below. These terms govern your use of our services and your relationship with Care Holistic.
2. Definitions
- “We”, “Our”, “Us” refers to Care Holistic
- “You”, “Your”, “Participant” refers to individuals receiving services
- “NDIS” refers to the National Disability Insurance Scheme (Australia)
3. Service Agreements
All services provided to NDIS participants must be governed by a signed Service Agreement that outlines:
- The type of supports to be provided
- Duration and frequency of services
- Pricing and billing terms
- Rights and responsibilities of both parties
4. NDIS Compliance
We are a registered NDIS provider and operate in accordance with the NDIS Code of Conduct, NDIS Practice Standards, and all applicable legislation.
Participants must have an active NDIS plan and funding available for the services requested.
5. Bookings, Cancellations & Changes
- We require at least 24 hours’ notice for cancellations or changes to scheduled services.
- Late cancellations or no-shows may be charged in accordance with the NDIS Pricing Arrangements and Guidelines.
- Any changes to services must be discussed and documented in an updated Service Agreement or Schedule of Supports.
6. Fees & Payment
- Prices are based on current NDIS pricing guidelines and may change in accordance with NDIA updates.
- Payments may be made through Plan Managed, Self Managed, or NDIA Managed funding arrangements.
- Invoices will be issued for services delivered, and payment terms will be stated in your Service Agreement.
7. Privacy & Confidentiality
Your personal information is protected under the Australian Privacy Act 1988. We will never share your data without your consent, except where required by law. For full details, see our Privacy Policy.
8. Participant Rights & Responsibilities
As a participant, you have the right to:
- Be treated with dignity, respect, and cultural sensitivity
- Access quality services that align with your goals
- Provide feedback or lodge complaints at any time
You are responsible for:
- Providing accurate information about your NDIS plan
- Attending scheduled appointments or giving notice
- Communicating changes to your health or circumstances

10. Limitation of Liability
Care Holistic is not liable for:

- Participant misuse or failure to follow agreed supports
